Children's Services

Communi-Care, LLC, is an approved provider for many of the services available to children and families enrolled in the Children’s Extensive Support (CES) Waiver and the Family Support Services Program (FSSP). Our providers are qualified as Personal Care Workers (PCW) under Communi-Care’s Non-Medical Class B license as a Home Care Agency through the Colorado Department of Public Health and Environment.

 

 

The services Communi-Care is approved to provide under these programs include:

 

Community Connector (CC) Services: This service supports the abilities and skills necessary to assist the child to access typical activities in the community and promote the child's interactions with non-disabled children and adults.

 

Respite Services: This service provides guardians a break from the demands of caring for a child with a disability with significant needs.

 

The waiver services enables children who would not otherwise qualify for Medicaid, due to income or assets, to access Medicaid benefits even if the child has private insurance. The target group for enrollment into the previous waivers are children who have intensive medical or behavioral needs due to the severity of their disability.

 

Children are assessed for their eligibility for the program through the Community Center Board.  In Pueblo, the Community Center Board options are Colorado Bluesky Enterprises (CBE): (719) 546-0572 or The Resource Exchange (TRE):  719-380-1100. The CCBs are responsible for assisting the family in developing a plan and securing the needed services that have been identified.

 

*The Family Support Services Program (FSSP) is similar to the other programs, but differs in some aspects. We would refer you to Colorado Bluesky Enterprises for enrollment into this program.

 

In order to qualify for services:

Children birth through age 4 must possess a developmental delay.  For ages 5 through 17, children must be diagnosed with a developmental disability.  Children age out of the waiver programs on their 18th birthday (*and FSSP at on their 21st birthday).
