Communi-Care is an approved service provider for the Supported Living Services (SLS) Waiver, Developmental Disabilities (DD) Waiver and the Omnibus Reconciliation Act of 1967 (OBRA) Waiver.

Our providers are qualified as Personal Care Workers (PCW) under Communi-Care’s Non-Medical Class B license as a Home Care Agency through the Colorado Department of Public Health and Environment.

The purpose of these programs is to provide persons with developmental disabilities, age 18 or older, tools to increase their ability to live independently or to increase their ability to function in their community with greater independence.

The Adult Services Communi-Care is approved to provide through the Division for Developmental Disabilities includes:

Personal Care Services: These types of services include but are not limited to the following:

     bathing, skin care, dressing, feeding, hair care, toileting, house keeping.

The extent of these types of services that may be provided as personal care must be considered non-medical or non-skilled. A referral to skilled care for CNA services can be made for individuals who require a higher level of intervention due to their condition.

Respite Care Services: Services that provide a break for primary caregivers of an adult with a developmental disability.  This service provided in the home or other locations in the community.   

 

Supported Community Connector Services: Supports the abilities and skills necessary to assist the person to access typical activities and functions of community life while developing and maintaining relationships and gaining greater independence. Activities in the community are designed to foster the  acquisition of social and adaptive skills, while building supports and relationships in the community.

Mentorship Services: Services that promote self-advocacy through instruction, modeling, and advising as well as providing experiences. 

 

Non-Medical Transportation Services: Services offered in order to enable the participant to gain access to Day Habilitation Services:  Specialized Habilitation or Supported Community Connector Services, as identified in their Service Plan.
